Includes space to complete Certificate of Inspection Number, Date of Test, Tested by, Contractor License Number and Appliance Code/s. Supplied in approved from as per clause 55(1)(a) of the Gas and Electricity (Consumer Safety) Regulation 2018. Must be affixed by a licensed gas fitter.
